Welcome to Counseling Services Stacey O. Kitchens, LLC. To ensure a seamless onboarding experience, please select the registration track below that corresponds to your appointment type.
Individual Private Practice & Telehealth Clients
If you are scheduling a traditional individual, family, or anger management session (in-office or via telehealth), please download and print the intake packets below:
For In-Office Appointments: Please bring your fully completed forms to your first scheduled session at our Covington office.
For Telehealth Appointments: Please securely scan and email your completed intake sheets to staceykitchens@outlook.com at least 24 hours prior to your virtual session.
Authorized Employee Assistance Program (EAP) Clients
If your counseling sessions are being fully covered by your employer’s EAP benefits, please submit the following paperwork before your initial intake:
Verification Reminder: Please ensure you have emailed your formal EAP Authorization Code, your EAP Vendor Network Name, and your approved session allotment to our administrative office to ensure zero-cost processing.
